Purpose:
Used to obtain demographic data and information on living arrangement before homelessness and other living conditions (i.e. education, employment, criminal justice contacts, out-of-home placement in childhood, income and entitlement support, current family support, and family history).

Housing status (Section A - #1-23): Questions assessing current living situation housing status in the past 7 days, including items on tenure, housing assistance, housing conditions, and services.

Housing history (Section A - #24-29; Section C - pages 36-39): Questions assessing housing history in the past 6-months, since age 18, and ever.

Description of the original study: Risk factors for long-term homelessness were examined among newly homeless men and women admitted to New York City shelters in 2001 and 2002. Interviews were conducted with 377 study participants upon entry into the shelter and at 6-month intervals for 18 months. In addition to the Community Care Schedule, standardized assessments of psychiatric diagnosis, symptoms, and coping skills; social and family history; and service use were analyzed.
