Data reporting tool utilized by the United States Department of Health and Human Services, Substance Abuse and Mental Health Services Administration service grantees to structure data they are required to report on the clients they serve in their funded programs.

Item description/response options:

Housing status (#1-5): Structured items assessing housing status in the past 30 days, including items on tenure, housing assistance and services.

Housing history (#6-10): Structured items assessing housing history since age 18, in past 3 years, and ever.
